Medicaid enhanced “Bump” rates effective through September 30, 2016

To qualify, Medicaid-enrolled primary care physicians must accurately self-attest by completing required paperwork as outlined below.

Necessary steps for Provider eligibility to receive the Primary Care Enhanced Physician Rates:

  1. Visit medicaid.alabama.gov
  2. Select Provider Enrollment from the drop-down menu under “Providers”
  3. Complete a self-attestation form
  4. Attach proof of board certification, if applicable
  5. Mail original self-attestation form to the address specified

Eligible providers will receive the Primary Care Enhanced Physician Rates beginning on when they complete the self-attestation form on file at HPES. If the provider has a form on file by October 31, 2015 they will receive payment at the enhanced rates retroactive to October 1, 2015. If the provider has a form on file November 1, 2015 or after, they will receive payment at the enhanced rate beginning with the date the form is entered into the HPES system.

The enhanced rates are approved for dates of service beginning October 1, 2015 through September 30, 2016.
