CMS has released the Preliminary Calendar Year (CY) 2026 Annual Performance Reports (APRs) for the expanded Home Health Value-Based Purchasing (HHVBP) Model through the iQIES system. The reports evaluate home health agencies’ performance during 2025 and show their Total Performance Scores, quality measure results, and Adjusted Payment Percentages (APPs), which will determine Medicare fee-for-service payment adjustments for 2027. Performance is measured across 10 quality measures covering patient outcomes, hospitalizations, discharge results, and patient experience. Eligible home health agencies can access their reports through the HHA Provider Preview Reports folder in iQIES using their assigned CMS Certification Number (CCN). CMS has also outlined the reconsideration and appeals process for agencies that previously submitted recalculation requests and disagree with the results. Agencies should review their reports, verify performance information, and pay attention to applicable deadlines, as payment adjustments can range from a 5% decrease to a 5% increase.
