CMS has released 2025 Merit-based Incentive Payment System (MIPS) final scores and performance feedback. Eligible clinicians and groups can sign in to the Quality Payment Program (QPP) website to review their scores and determine their 2027 Medicare payment adjustment.
- Above 75 points: Positive payment adjustment.
- Exactly 75 points: Neutral payment adjustment (0%).
- Below 75 points: Negative payment adjustment.
Targeted Reviews
Clinicians, groups, subgroups, virtual groups, APM Entities and authorized representatives can request a targeted review if they believe an error affected their final score or payment adjustment. Examples include data submitted under the wrong Taxpayer Identification Number (TIN) or National Provider Identifier (NPI), incorrect Qualifying APM Participant (QP) status, or failure to automatically reweight performance categories when eligible.
The targeted review period closes November 30, 2026, at 8 p.m. ET. CMS may require supporting documentation, and approved requests that result in a scoring change will be reflected in updated scores or payment adjustments as soon as technically feasible. Targeted review decisions are final and cannot be appealed through another review.
Providers should review their final scores and performance feedback promptly to identify potential errors before the deadline.
