CMS has released 2025 MIPS performance feedback and final scores through the Quality Payment Program (QPP) website. The 2025 final score will determine the MIPS payment adjustment applied in 2027, with CMS expected to release information about the 2027 payment adjustments in approximately one month.
Clinicians and organizations can sign in to the QPP website using their Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) credentials to review their feedback. After signing in, users can select “View Feedback” and choose the applicable organization, such as a practice, APM Entity, or virtual group. Practice representatives can access individual, subgroup, and group feedback, while third-party representatives cannot access final feedback or payment adjustment information.
Medicare Shared Savings Program ACOs are encouraged to have at least one individual obtain a HARP account with the Security Official role. Additional individuals may request Staff User access through the ACO Management System (ACO-MS) to manage QPP access and review applicable information.
CMS is also offering a targeted review process for participants who believe there may be an error in their 2025 MIPS final score. Examples include data being submitted under the wrong TIN or NPI, having Qualifying APM Participant (QP) status but receiving a MIPS payment adjustment, or not receiving an applicable performance category reweighting due to extreme and uncontrollable circumstances.
The targeted review period will remain open for approximately 60 days. Eligible participants can request a review through the QPP website by selecting “Targeted Review” after signing in. CMS may request supporting documentation, and if a review results in a scoring change, the final score and applicable payment adjustment will be updated. Targeted review decisions are final and cannot be appealed through another review.
CMS has also released resources covering the 2025 MIPS performance feedback and targeted review processes, including guidance on available feedback, supplemental reports, and circumstances that may qualify for a targeted review.
