Anesthesia groups face mounting pressure heading into 2026

As anesthesia practices prepare for 2026, the specialty is confronting increasing financial and operational strain driven by reimbursement cuts, workforce shortages, and rising costs. Five key trends are shaping the outlook:

  1. Reimbursement continues to decline in real terms.
    CMS’ 2025 anesthesia conversion factor dropped another 2.2%, continuing a multiyear trend. Average anesthesia payment has fallen from $22.27 per unit (2019) to $20.44 (2024), meaning compensation isn’t keeping pace with inflation or operating expenses.
  2. Higher dissatisfaction across anesthesia professionals.
    Growing clinical demands, heavier call obligations, and expanded perioperative roles—without corresponding reimbursement—have led to rising burnout. More than 40% of anesthesiologists and CRNAs say they may leave their current roles within the next two years.
  3. A worsening national workforce shortage.
    Projections show a shortage of 6,300 anesthesiologists by 2036, and more than half of currently practicing anesthesiologists are over age 55. Without changes, many groups may face challenges maintaining OR coverage and supporting ASC growth.
  4. More payer pressure and reimbursement erosion.
    In addition to CMS cuts, anesthesia groups report increasingly aggressive commercial payer policies—lower allowable rates, more denials, and heavier administrative demands. Practices are struggling to balance this with rising labor, technology, and overhead expenses.
  5. Consolidation and contracting volatility.
    Ongoing consolidation among hospital systems and large anesthesia organizations is reshaping the market. Independent groups have less leverage, and more frequent contract shifts are adding uncertainty for both practices and facilities.
