Released on December 22, 2025, the specifications allow clinicians, groups, virtual groups, subgroups, and APM Entities—including Medicare Shared Savings Program ACOs—to identify QCDR measures that align with their specialty and reporting needs.
The file includes a full list of CMS-approved QCDR measures and identifies which CMS-approved QCDRs are authorized to submit each measure for the 2026 MIPS performance period. Because QCDR measures can only be reported through a CMS-approved QCDR—not a Qualified Registry—reviewing this information now can help organizations plan ahead and avoid reporting gaps.
