CMS Releases Key Guidance on Upcoming Medicaid Eligibility Changes for Noncitizens

The Centers for Medicare & Medicaid Services (CMS) has issued new operational guidance to help states prepare for significant Medicaid eligibility changes tied to H.R. 1, set to take effect October 1. The policy will narrow federal Medicaid and CHIP funding to specific categories of noncitizens, requiring states to make timely updates across eligibility systems, applications, verification processes, and claims infrastructure.

Under the new rules, several groups previously eligible for full Medicaid coverage will no longer qualify for federally funded benefits. However, emergency services will continue to receive federal support, and states may still choose to cover certain populations, such as children and pregnant women, under existing options.

CMS is also directing states to review financial and operational impacts, including potential adjustments to managed care payment structures and contract requirements. The agency has already identified substantial spending tied to noncitizen coverage in select states and plans to continue oversight efforts moving forward.

To remain compliant, states must meet upcoming deadlines for submitting required plan amendments, making it critical for stakeholders to review CMS guidance and assess how these changes may affect coverage, reimbursement, and program administration.
