UnitedHealthcare Expands Doula Coverage to Support Maternal Health Outcomes

UnitedHealthcare has announced a nationwide expansion of doula coverage for eligible employer-sponsored and commercial plans, marking one of the greatest efforts of its kind in the private insurance market.

The phased rollout, beginning in 2026, could reach approximately 7.2 million members by January 1, 2027, depending on employer participation. Coverage options may include a set number of doula visits or reimbursement allowances, with services available both in-person and virtually. Members may also have flexibility in selecting doulas, regardless of network status.

This expansion comes amid ongoing concerns about worsening maternal health outcomes in the U.S. Research cited by the insurer suggests that doula support can lead to improved outcomes, including reductions in preterm births, cesarean section rates, NICU admissions, and postpartum mental health conditions.

In addition to expanding coverage, the United Health Foundation has committed funding toward maternal and infant health initiatives, including efforts to grow and diversify the doula workforce.

For employers and providers, this move signals a growing emphasis on supportive care models aimed at improving maternal health outcomes and enhancing patient experience.
