Practices can use patient-reported blood pressure measurements to help in closing gaps in care, as well as assist with telehealth encounters. See the tips below as to best practices.
- Blood pressure measurements may be submitted on the same day as an office visit or submitted at a later time.
- Blood pressure measurements may be obtained through various methods including telemedicine, telehealth, e-visits, virtual or phone encounters, as well as from a patient’s home log brought in during a regular, in-person visit.
- Practices may submit the blood pressure measurements alone when necessary if taken outside of the provider’s office.
- Digital blood pressure devices are required, but are not necessary to document in the medical record.
- Documentation in the medical record must include the measurement, the date and that the measurement was taken by the patient.
- When multiple blood pressure measurements are taken on the same day, practices should submit the codes for the lowest systolic and the lowest diastolic reading.
- Practices should use the Provider Insights Opportunity List to identify patients who have a gap in care for this measure regarding blood pressure.
